Step 1:  Create your Account

This quick form will request basic business information such as your EIN, web address and contact information.

Step 2: Complete the Eligibility Screening

Answer a short series of questions to confirm your eligibility. Once eligibility is confirmed you’ll receive an invitation to apply for the grant.

Step 3: Gather Required Materials

  • Two Profit & Loss statements (pre- and post-disaster)
  • Business license or proof of ownership
  • W-9 form
  • Photos or other proof of disaster impact
  • If applicable, repair estimates or invoices

Step 4: Complete the Full Application

Provide detailed business and disaster information including recovery plans, upload required documents, and review demographic questions (optional).

 Step 5: Submit and Receive Confirmation

After submission, you’ll receive a confirmation email. Your application will be reviewed, and you’ll be contacted if more information is needed.
